• Athletic Pre-Participation Physical Evaluation Form PART B – This must be filled out by your doctor and is valid for one year. This form will be kept on file in the nurse’s office and must only be filled out once per year.
The following forms must beCompleted and signed for each season in which your student participates in a sport.
• Health Questionnaire PART A – To be filled out by parent; this is to be submitted with a "NEW" physical exam.
• Athletic Health History Update - To be completed if your child has a current physical exam already on file in the FMS health office. This takes the place of "PART A".
• Emergency Card – Please fill out completely – front and back
• Athletic Participation Approval – Must be signed by parent and student
When a new part B physical exam form is submitted, part A parent questionnaire must accompany the physical form. If your child participates in an additonal sport during the school year, the parent may use the Athletic Health History Update form rather than completing part A again.
