NOTICE REGARDING WINTER
SPORTS FOR PARENTS
Girls-Basketball,
Cheerleading
Boys-Basketball, Wrestling
•
A physical exam that was done on or after 12-1-08 can be accepted for
winter sports in the 2009/2010 year
• A physical is good for one calendar year.
• A new part A form, yellow and green card must be completed for
each new sport.

Make sure that vision, BP, and pulse are recorded on the form. INCOMPLETE FORMS CANNOT BE ACCEPTED.
Yellow and green cards are available outside of the main office and in the nurses’ office
The nurses will start processing forms for winter sports on Monday, 11-9-09

Since
we have added a fifth sport to our spring season this year and only
have 4 indoor coaching locations within our two gymnasiums there is
a need to implement a rotating schedule to accommodate each sport. Please note the following:

If the weather and field conditions do not cooperate,
the indicated team on the calendar will Not be practicing on that specific day, and those students will
go home on the 2:18 bus.
· If weather permits and we can get outside, the team that has off on the calendar may still hold practice. If this decision is made the following communication will happen by 1:00 p.m.:
1. An announcement to the students will be made via the main office.

The change will be posted on FMS website.

Please have your child bring their sports gear
every day.
